SMHPS Registration

Acceptance on to the SMHPS Register is by application and submission of the audit tool, which has been completed within the previous 6 months. You need only submit a completed word OR excel audit tool, not both.

A blank audit form and a completed Exemplar (with associated Record of Evidence) are provided below.

Once the audit is completed, please fill out the registration details (see link above) and upload your completed audit form. The submitted audit form will be checked to ensure all sections have been completed, after which the administration details completed by you will be entered into the SMHPS Register.
